My E-signature below indicates my permission for LifeServe Blood Center to use photographs or video of me for purposes approved by LifeServe Blood Center. I consent to release LifeServe Blood Center and its agents from any and all liabilities for use of video or photographs taken for publicity purposes. I give permission for my image to appear on blood donation promotional materials including but not limited to: bloodmobile buses, commercials, printed materials, flyers, mailing materials, and online. I also authorize the release of my name with any such photography or video images.
